Information for New Clients
Appointments can be scheduled by phone, email, or online. It is highly recommended that you book your assessment and two treatments in advance to ensure you can access the times that work best for you.
Every effort is made to start and end appointments on time. If you are late for your appointment, your time with the therapist will be shortened to avoid delaying those scheduled after you.
A minimum of 24 hours notice is required to cancel an appointment. There will be a fee of $45.00 charged for a late cancellation or missed appointment. This amount is to be paid before any further appointments can be booked.
Please arrive 5-10 minutes before your scheduled first appointment. Your first appointment will last 60 minutes and all appointments are offered in a private room.
After booking your first appointment your intake, medical history and consent forms will be emailed to you from our booking program JANE. In order to maximize your assessment time, please fill out all forms in advance ONLINE. It is also important that you understand and are able to consider and consent to your treatment, so please review the consent forms prior to your arrival.
You can wait to sign the forms with your therapist after you have had a chance to ask any questions.
If you DID NOT receive access to the online intake forms, please email: firstname.lastname@example.org.
Physiotherapy treatment is normally included within employer-paid or individually purchased health benefit plans. Therefore, your individual extended health plan may cover part/all of your physiotherapy. You are required to pay for your treatment in full on the day of the service; a receipt for the services rendered will be issued at the time of your visit and can be submitted to your extended health plan. We accept cash, email money transfer, debit (Interac), VISA or Mastercard.
All appointments are usually scheduled for 1/2 hour and no other clients are booked in your time. We only see one client at a time.
